Entry ordering that every local exchange carrier required to provide TSA as of December 1999, that has neither init- iated the customer notification efforts nor committed to sustain the $3.50 monthly discount shall issue customer notification in accordance with the guidelines set forth within Findings 3 and 6 of this entry; that every local exchange carrier required to provide TSA as of December 1999, that has not yet filed requisite revisions to company tariffs must do so pursuant to the guidelines set forth within Finding 7 of this entry; that, effective immediately, all TSA-related monthly and annual advertisement and/or notification requirements previously promulgated under Case No. 91-564-TP-UNC are hereby repealed; that every local exchange carrier required to provide TSA during the six- month period beginning with July 1, 1999, and ending with the program's sunset date of December 31, 1999, shall file its final TSA annual report for that period no later than September 1, 2000.
