Entry ordering that consistent with Finding (4), each Ohio rural carrier that wishes to be eligible for one or more of the high cost support mechanisms identified in Finding (2), shall file with the Commission a completed and notarized copy of the attached affidavit form, certifying that federal high cost support is being used consistent with Section 254(e), by no later than September 14, 2009. |
