Entry ordering that the secretary of the Commission send to all public utilities a copy of this entry and two copies of the appropriate form of the annual report (or "Assess- ment Report" to CMRS providers) to be completed by the public utilities; that the public utilities shall complete and return the annual report (or "Assessment Report" to CMRS providers) by no later than March 31, 2000, to the the Puclic Utilities Commission of Ohio and a copy with the Ohio Consumers' Counsel; that the secretary of the Commis- sion send to all railraod companies a copy of this entry and two copies of the appropriate form of the annual report to be completed by the railroad; that the railroads shall com- plete and return the annual report by no later than March 31, 2000, to the Public Utilities Commission of Ohio.
