Entry ordered; that consistent with Finding 5, each Ohio non-rural carrier that anticipates funding for the year 2006 from the federal hold-harmless provision identified in Finding 2, and each rural carrier that wishes to be eligible for one or more of the high cost support mechanisms identified in Finding 3, shall file with the Commission a completed and notarized copy of the appropriate attached affidavit form, certifying that federal high cost support is being used consistent with Section 254(e), by no later than September 7, 2005. |
