Entry ordering that Ameritech's August 17, 1999 application for temporary waiver is granted, subject to the additional conditions set forth in this entry; that Ameritech shall place all involuntary universally toll blocked customers into a dePICed status as soon as possible, and in any event, by April 1, 2000; that Ameritech shall provide a customer notice by December 1, 1999; that Ameritech shall work with the Commission's staff, and obtain its prior approval, in coming up with the language which shall be included in the required customer notice.
