Finding & Order that, with the exceptions of the implementa- tion cost recovery MOU rate, and the proposed intraLATA presubscription charges, the proposed application of the Applicant to amend it tariff is approved; that the Appli- cant's request to charge a Subsequent Service Order charge of $8.00 and a Central Office Connection Charge of $15.00 is denied; Applicant is directed to limit its intraLATA presubscription charges to $5.00 for the first line and $1.50 for each additional line presubscribed at the same time; that Applicant file its proposed MOU rates for in- cremental cost recovery no later than August 24, 1998; the recovery mechanism will become effective on the 31st day after Applicant files its proposed MOU rate and this case shall be automatically closed of record, unless otherwise acted upon by the Commission.
