Entry ordering that the recovery of omission fees levied in accordance with Section 3745.111, Revised Code, and paragraphs (C) and (D) of Section 3745.11, Revised Code, shall take the form of a base rider, to be in the form of Attachment A to this entry; that the utility shall provide with its application the cost data and information regarding the derivation of the proposed rider, in accordance with Attachment B to this entry; that unless, upon review, the Commission orders an investigation and/or hearing suspending the application, the rider shall take effect on the 46th day following its tariff.
