In the matter of the application of Broadslate Networks of Ohio, Inc. to make changes to its tariff, PUCO No. 1, to update the tariff checklist; revise service name and add new service offering to table of contents; revise service name in Section 3.2.3 and introduce new service offering as described in Section 3.2.4; and to revise service name and reduce certain rates in Section 4.2, and intoduce pricing for new service offering in Section 4.3.
