Entry ordering that all telephone companies utilize the telecommunications application form and the ACE supplemental application form in accordance with Finding (2); that all telephone companies currently providing tariffed Tier 2 nonresidential services and all telephone companies providing regulated toll services shall file the applicable detariffing ATA in accordance with Findings (3) and (4); that all telephone companies seeking the detariffing of Tier 2 nonresidential services and regulated to all services must provide customer notice in accordance with Finding (3); that in conjunction with the provision of customer notice, all telephone companies must file the requisite affidavit in accordance with Findings (3) and (5). |
