Entry ordering that Certificate Nos. 69120-I, 4024-I, and Permit No. 50438 issued to Jones are reinstated, but all operations under such authorities are sus- pended for a period of 180 days; no further extensions of temporary authority shall be granted; that Jones purchase an annual decal for 1993 within 60 days and continue to purchase an annual tax decal during the term of suspension; that Jones' authority shall be subject to revocation if the required certificates of insurance are not filed and operations are not re- activated by the end of the period of suspension; that Jones shall no be required to maintain insurance coverage during the term of suspension.
