Entry ordering that, effective January 1, 1999, the pro- hibition promulgated by the Commission on December 23, 1997, which prevents a telephone customer from simultaneously participating in TSA and the federal Lifeline program shall be repealed; that, all carriers currently required to pro- vide TSA are directed to insert the necessary company- specific information into the language included as Attach- ment A to this entry, and to file the two revised TSA tariff sections by January 4, 1999; that, beginning January 1, 1999 the TSA customer notice required by Section 4905.76(D), Re- vised Code, and included as Attachment C to the PUCO's June 20, 1991 Finding and Order, shall be revised in accordance with Finding 8.
