Entry ordering that, effective no later than 10/30/95, gas, natural gas, or electric light companies under the PUCO jurisdiction restore service pursuant to the terms set out herein, of those whose gas or electric utility service has been disconnected for non-payment; that the requirement set forth in Rule 4901:1-18-04(B)(5), O.A.C. be suspended until 7/1/96, except for those PIPP customers individually contacted by a provider affiliated with a utility or a Department of Development sponsored weatherization program; that each gas, natural gas, and electric light company under the PUCO jurisdiction provide, by county, the name, address, and the telephone number, if available, of each of its current PIPP customers as well as monthly and annual consumption data for these customers on a timely basis to the Ohio Department of Development; that each gas, natural gas, and electric light company under the PUCO jurisdiction provide the data requested in the Special Data Request, attached to this entry as Appendix A, at the place and the times set forth in the request.
