Entry ordering that Columbia Gas of Ohio submit no later than 8/26/94 the information required by the Standard Filing Requirements on the basis of a test year beginning November 1, 1993, and ending October 31, 1994, and a date certain of December 31, 1993; that the appropriate method for making any changes to the date certain or test period shall be the filing of a new notice of intent to file an application for an increase in rates and withdrawal of the pending application; that the requests for waivers made by the applicant are granted as set forth in Finding 6; that the request to incorporate by reference and adopt the Standard Filing Requirements filed in Case No. 94-987-GA-AIR is reasonable and should be granted.
