Entry ordering that, consistent with Finding 5, each Ohio non-rural carrier that anticipates funding for the year 2003 from the federal hold-harmless provision identified in Finding 2, and each rural carrier that wishes to be eligible for one or more of the high cost support mechanisms identified in Finding 3 shall file with the Commission a completed and notarized copy of the attached affidavit form, certifying that federal high cost support is being used consistent with section 254(e). |
